They will be glad to assist you.  Orders may be submitted: By telephone with credit card payment information By filling out the SISU/Kol –Ami order formand sending it in with a check By sending an official Purchase Order form or letter on official school letterhead to be invoiced. If your institution is in New York State, please include a copy of your Tax Exemption certificate. Orders From Outside of North America Orders from any country outside of North America are handled directly by JAFI. Order Form Ordering Additional Items from the Kit: Individual additional items from the kit are only available to schools that have already purchased a kit. Order Form - additional items Payment can be made by check, credit card or bank transfer. Upon receipt of your filled-in order form together with payment we will send you the kit or additional items by airmail.
